In order to become a client in a professional counseling relationship with me, the state of Colorado will require that you read, understand and sign a Client Information Form and a Disclosure Form, both of which are HIPPA compliant.  If you would like to obtain a copy of either form, please contact me and I will be happy to send them to you to look over and consider.  

Note:  Please be advised that as of June 1, 2022, I have updated my disclosure statement to include a policy regarding legal proceedings.  It is important that all clients understand that there is a significant financial cost for any involvement I am called into in court matters.  I am bound by HIPPA and ethical constraints, and being called into court proceedings is not the focus of my career.  I have a fee schedule for all time/effort/costs involved should I be called into court.
